3. • Log on to our website at fcnvaccinationcenter.com, click "BOOK APPOINTMENT" and fill up all the details needed. 
4. Wait for the confirmation of the appointment. "Strictly No Walk In."
5. Go to the FCN Medical & Vaccination Center at 6th Flr. Rm. 601 San Luis Terraces 638 T.M. Kalaw Ave. Ermita, Manila on the date of approved appointment.
6. Present the PROMO CODE provided by Allianz PNB LIfe Inc. with valid ID. FCN will verify if the Promo Code is under the client's name.
7. Fill up the form and return to the person in charge with complete details.
8. Wait for you turn. Present the Form, Promo Code, and Valid ID
