Follow the procedure below:

1. Present your Healthbox Digital ID and government-issued ID to be used (front portion) for verification in order to avail of the promo offer.
2. Place the subject line as: Allianz PNB Life/Name/Check-in date (e.g. BPI Deals Promo/Juan Dela Cruz/05-10-2021)
3. Upon verification, Nurture Wellness Village shall send a Statement of Account with payment link and instructions via e-mail.
4. Full payment via payment link within the specified period is required to confirm the booking, otherwise, the Hotel reserves the right to cancel the booking.
5. Once full payment has been made, the hotel will send confirmation voucher via e-mail
6. The confirmation voucher must be presented upon checking-in together with valid IDs for all guests.

• Offer is valid from Monday to Thursday only and 1 transaction per member only.
• Stay Period is until July 30, 2025.
• Prior reservation is required and shall be made two (2) weeks ahead of the preferred stay date.
• All reservations are considered tentative until Nurture Wellness Village receives the full payment.
• The booking made is non-refundable & cannot be modified.
• Black-out dates: Fridays to Saturdays, Regular, Public & Legal Holidays
• Members should present a proof of qualification to the staff to avail the offers.
• Promo cannot be in conjunction with other promo and discounts such as Senior and PWD.
